About the Role

 

The Community Nurse provides and coordinates palliative care for patients under Hospice Malta’s care within their own homes, supporting patients and their families through incurable illness and end of life. The role contributes to Hospice Malta’s mission of delivering the highest standards of palliative care with compassion and dignity.

Duties and Responsibilities

Reporting to the Community Coordinator, the Community Nurse delivers palliative nursing care to patients in their own homes across Malta. Key responsibilities include:

Assessing, planning and implementing individualised palliative care plans for patients in the community, addressing their physical, psycho-social and spiritual needs.

Carrying out regular home visits to monitor patients’ conditions, review care plans and provide compassionate nursing care and support.

Maintaining accurate and up-to-date records of assessments, home visits and telephone contacts with patients and families.

Liaising with and referring to other healthcare professionals or agencies as necessary to ensure coordinated patient care.

Participating in and contributing to multidisciplinary team meetings, discussing and reviewing patient care plans.

Promoting awareness and understanding of palliative care within the team and the wider community.

Making efficient and effective use of resources, including responsibility for the control and stocking of clinical equipment and supplies.

Reporting any potential or actual problems or complaints promptly to the Community Coordinator.

Complying with all Hospice Malta policies, procedures, and professional standards.

Upholding the dignity, rights, and confidentiality of patients and their families at all times.

Undertaking any other duties commensurate with the role, as reasonably directed by management.

Qualifications and Experience

Mandatory

Sc. in Nursing / SRN qualification, with a valid warrant issued by the Malta Nursing and Midwifery Council. Foreign qualifications must be accompanied by MQRIC recognition issued by MFHEA.

Minimum 2 years’ post-registration nursing experience.

Valid nursing warrant / professional registration.

Valid Category B driving licence.

Fluency in spoken and written Maltese and English.

Desirable

Postgraduate qualification or specialised training in palliative/hospice care.

Contract, Location and Benefits

Full-time, indefinite position; community-based across Malta, reporting base at St Michael Hospice, Santa Venera.
